Five Facet Mindfulness Questionnaire - This webpage on the Positive Psychology website describes the validity, scoring, and different versions of a questionnaire that can be used to assess mindfulness.
Mindfulness-Based Relapse Prevention Adherence and Competence Scale (MBRP-AC) - This scale can be used to rate therapist adherence and skill when delivering mindfulness-based relapse prevention. The scale is located in the Appendix of this article.
