Patient perceptions of contingency Management serve as both barriers and facilitators, as some patients may find CM to be overly focused on abstinence, and some may view it positively.

Barriers

When protocols focus on abstinence, some patients may not appreciate the repeated need for toxicology screens. When Contingency Management (CM) is delivered virtually or through an app, patients may find the need to complete random screens particularly inconvenient.

Patients with limited transportation or childcare options may find it challenging to attend multiple CM sessions per week. For this reason, CM is often delivered in settings where patients are already coming frequently (intensive outpatient, partial hospitalization, opioid treatment programs that dispense methadone and buprenorphine).

Facilitators

Generally, patients have a positive view of CM and feel it supports them in reaching their treatment goals

The process of earning incentives not only makes patients feel good in the moment, but also helps them earn bigger and bigger incentives, which can foster a sense of independence as they are able to afford things for themselves and others.
